Information - Freshmen & Transfers

Daytona State College uses a secure Electronic Medical Records System called Magnus Health to collect and store all health information. You will receive a username and password directly from Magnus Health to your FalconMail account (When you were admitted to Daytona State College, an email was sent to you with your FalconMail username.)

Carefully read all of the following information before completing the necessary forms on the Magnus Health website. All forms and any required documentation must be submitted by the due date, or you will not be allowed to practice or take part in conditioning. You will be asked to sign a statement saying that you have read and understand our policies. A parent or legal guardian must sign this Procedures document if you are covered under their health insurance plan. Parents must complete and sign all forms for any athlete under 18 years of age.

PREVIOUS INJURIES

All student-athletes must provide documentation from a physician regarding current and/or previous history of medical conditions, including concussion, cardiac incidents, surgeries, and all other injuries. If you are currently under the care of a physician or undergoing physical therapy or rehabilitation for an injury, you must submit all paperwork associated with that injury, including physical therapy discharge notes, in order to be eligible to begin participating in any athletic activity at Daytona State College. Written clearance from a medical doctor to participate in athletics, written on his/her letterhead, must be provided for any injury incurred within the last year for which a doctor was consulted and/or which prevented you from participating in your sport.

COPY OF CURRENT INSURANCE CARD

You must upload a copy of the front and back of all health insurance cards to the designated location on the Magnus Health website. Please include general medical, dental, and vision plans, if applicable. If the student-athlete’s insurance changes during the school year a new copy must be submitted, and the new information provided by logging in to the Magnus website.

SICKLE CELL TESTING

All student-athletes must show proof of sickle cell trait status or be tested at Daytona State College on the day of the physical. Instructions for uploading documents are provided on the Magnus Health website.
ATHLETE PHYSICAL EXAMINATIONS

All student athletes must have an athletic physical examination conducted by DSC Medical Staff before participating in any College-related athletic activity. Examinations are conducted at Halifax Sports Medicine Clinic or in the DSC Athletic Training Clinic on assigned days and times after arrival on campus. Students should check with their head coach as to the date and time of their exam. Failure to show for an assigned physical will result in the student having to pay for a physical arranged by the Office of Sports Medicine with our team physicians during their office hours.

Students who have not been cleared by the DSC team physician may NOT play or practice with the team until requirements have been met – this may be additional medical testing, or the submission of documentation. Testing related to pre-existing conditions discovered during the physical exam must be done at the expense of the student-athlete.

INTERCOLLEGIATE ATHLETIC ACCIDENT INSURANCE COVERAGE

PRIMARY INSURANCE

Daytona State College does not cover expenses related to illness or for any injury that isnot related to DSC-organized sport participation.

We highly recommend that each student-athlete carry private health insurance. We also recommend that any athlete with a managed care (HMO/PPO) policy who resides more than 30-45 minutes away from Daytona Beach change his/her primary care physician to a local one, preferably one of Daytona State College’s primary care physicians. This change requires a simple phone call to your insurance company and will decrease the amount of time, money, and energy you will have to spend on accessing appropriate health care, and therefore, time away from active sport participation.

The DSC Sports Medicine Department must receive any changes to an athlete’s health insurance coverage within 30 days of that change. If proper notification is not received, Daytona State College Athletics will not be responsible for any resulting delays in treatment, payment, collection notices, etc.

SPORT-RELATED INJURY INSURANCE COVERAGE

Athletic Accident Insurance is provided by Daytona State College through Ascension Insurance for the benefit of our student-athletes. This coverage is offered on a “Full Excess” basis only. Under the terms of the policy, the coverage is considered secondary payor to all other valid and collectible medical insurance policies, and covers only injuries/illness/accidents that are a direct result of participation in the intercollegiate athletics program during the dates of the primary competitive season and designated off-season conditioning sessions. Health care providers will bill your insurance first, and then submit the balance to Ascension. Balances remaining after being processed by Ascension will be the responsibility of the student-athlete.

The Athletic Accident Insurance underwritten by Ascension is comprehensive, but it has standard limitations, some of which are listed below:

1. It does not cover expenses due to illness.
2. It does not cover injuries incurred while participating in a non- “countable” Daytona State College sport-related activity, such as lifting weights or conditioning on your own.
3. It does not cover expenses incurred from “off-campus” Physicians/Medical Specialists unless there is a written referral, pre-approved by Daytona State College’s Team Physician and the Director of Sports Medicine. It does not cover expenses related to pre-existing injury or illness.
4. It does not cover physician visits, diagnostic testing, or rehabilitation requested by Daytona State College Team Physicians during the pre-participation examination
5. All surgical treatments must have prior written authorization.
6. Only those bills incurred within a period of three years from the date of the injury are covered.
7. Only dental injuries that are incurred while participating in an official practice or game are covered; no other dental care is provided under this policy.
8. Claims sent to Ascension/Summit must be initiated at the time of injury and will not be paid unless and until Ascension receives an Explanation of Benefits by the athlete’s primary insurance carrier.

MEDICAL BILLS

Health care providers do not send bills to Daytona State College. If you receive a bill or statement for an injury that occurred as a direct result of participation in intercollegiate athletics at Daytona State College, you must submit the bill/statement to a DSC athletic trainer within 30 days of receipt. Bills not received in a timely manner will result in the student-athlete being financially responsible.

Daytona State College will not be responsible for payment of any medical bills if the above procedures are not followed. Any questions about a claim should be referred to the Office of Sports Medicine without delay.

 

SPORT-RELATED INJURY OR ILLNESS

REPORTING PROCEDURES

All injuries/illnesses must be reported to a member of the Sports Medicine staff immediately. Many injuries can be diagnosed and treated in the Athletic Training Clinic by Certified Athletic Trainers. If the athletic trainer determines that a referral to a physician or other medical provider is warranted, the Sports Medicine staff will make all of the necessary arrangements and/or referrals. In the event of an emergency due to an athletic injury or illness, the student-athlete will be transported to a pre-arranged hospital or medical facility. If the student-athlete is evaluated or treated by someone other than Daytona State College’s Sports Medicine staff, he/she must provide our office with written documentation regarding the condition, including clearance to participate in his/her sport, prior to return to athletic activity.

The athlete should at no time seek outside medical attention for an athletic-related injury or illness without prior written authorization from the Director of Sports Medicine unless the injury results in a medical emergency. Failure to follow these procedures will result in the student-athlete being ineligible to play or practice with the team until all related medical records/documentation are received and reviewed by the Sports Medicine Office, and will result in the student-athlete being financially responsible for resulting medical bills.

Daytona State College and the Athletic Department shall not be responsible for any charges incurred due to examination, tests, treatments, and/or surgeries by physicians, consultants, and/or hospitals if these procedures are not followed.

TREATMENTS/REHABILITATION

The Athletic Training Clinic is located in the Lemerand Center. Clinic hours are posted with each sport season. All student-athletes are expected to adhere to the Athletic Training Clinic Rules which are reviewed during orientation and are posted in the clinic. Treatment/rehabilitation and taping will be available to all athletes, but those in-season will receive first priority. Failure of an injured athlete to keep treatment and/or rehabilitation appointments will be interpreted as the athlete’s unwillingness to cooperate with the Sports Medicine Staff for the earliest possible return to competition and may result in the student-athlete being ineligible to participate in any Daytona State College athletic activity. The head coach will be informed if athletes fail to keep appointments.

In some cases it may be necessary for the athlete to receive rehabilitation at a physical therapy clinic off campus. If the injury being rehabilitated occurred during participation in a DSC-sanctioned sporting event, DSC’s secondary insurance policy will be billed for any amount not covered by the athlete’s primary policy.

PRACTICE OR GAME PARTICIPATION FOR AN INJURED OR ILL ATHLETE

Decisions about the availability of an athlete for practice or game competition shall be the sole responsibility of the members of the Daytona State College Sports Medicine Staff. A student-athlete who is feeling too ill or injured to participate MUST contact the athletic training staff prior to that day’s practice. The final decision as to whether a student-athlete may participate in any varsity sport activity rests with Daytona State College’s Team Physician.
